About This Site

This site is an independent education project. It has one job: to turn the medicines you type into a clean sheet you can print or save as a PDF.

Who runs it

It is run independently. It is not a pharmacy, a clinic, a care agency or a store. It does not sell anything, and it does not take payment, ads or sponsorship.

What it does

  • It lays out the medicines, amounts and times you type on a daily, weekly or monthly sheet, or on a medication list.
  • It adds boxes to check, initials columns, an optional injection site column and lines for notes.
  • It prints from your browser. "Save as PDF" uses your browser's own print window.

What it does not do

  • Every word on your sheet is one you typed. The site has no list of medicines, amounts, times or body sites to offer, and it will not fill one in for you.
  • Nothing here is medical advice. Your label, your doctor and your pharmacist come first.
  • The home MAR is a family tool. Care homes and agencies should use the form their rules require.

How it is tested

Before launch, every layout was printed to PDF from Chromium (the open source base of Chrome) on US Letter and A4 paper on 4 October 2026, with test medicines typed in. On a weekly sheet, we checked that the medicines we typed showed up in print. We also checked that the weekly and monthly sheets print sideways. Then we counted how many medicines fit on one page. Each tool page shows that count.

Where the facts come from

Advice about keeping a medication list is taken from two US government pages for consumers, one from the FDA and one from AHRQ. They are named at the foot of each page, with the date they were read.
