What a MAR sheet is
MAR is short for medication administration record. It is a grid. The medicines go down the left side and the days of the month go across the top. Each time someone gives a medicine, they write their initials in that day's box.
The home MAR on this page adds an initials key at the bottom. Each helper writes their initials and full name once, so anyone can see who gave what.
This MAR is for home use
This template is for families and friends who help someone at home. It does not claim to meet any state, agency or facility rule.
Care homes, home care agencies, group homes, schools and camps often have to use a set form. If you work for one of them, use the form your state, agency or facility gives you.
How to fill in a home MAR
- Type each medicine and its amount exactly as the label says.
- Type the time from the label. If a medicine is given more than once a day, give each time its own row.
- Pick the month in "Start date". The sheet shows every day of that month.
- Print it, and fill in the initials key at the bottom.
- Each time you give a medicine, write your initials in that day's box.
- Use the notes lines for anything the next helper should know.
If you track shots, turn on the injection site column. Each medicine then gets a second row to write the spot used.
How many medicines fit on one page
The monthly MAR packs a box for every day of the month into each row, so it holds fewer medicines. Our 4 October 2026 print test in Chromium (the open source base of Chrome), with 3 blank rows, gave these counts:
